Children under the age of 16: Must be accompanied
by both parents at the post office or at the courthouse to sign the application.
Parents must submit identification and evidence of relationship to the child (ie
child's certified birth certificate). There are exceptions to this requirement,
please call us for detailed instructions.
For children 16-17 years of age: It is preferred to
have at least one parent accompany them to co-sign the application at the post office
or the courthouse.
Adults 18 years and older: Can sign for themselves.
Please bring the following items to our office at 1107 Rio Grande St. before
3pm so you may meet the 4pm
deadline at the post office or the courthouse. Directions
