for passport renewal, please refer to

2) Proof of name change if any: (Certified marriage license, divorce
decree, etc.)

3) Proof of Departure: Copy of airline ticket or travel itinerary or original
letter from your
company (For sample letter click here
).

or money order made out to Department of State).
