Proof of Citizenship

  • ORIGINAL certified Birth Certificate, issued by the State, City, or County bearing the official seal (or)
  • Certificate of Citizenship or Naturalization (or)
  • Certification or report of Birth Abroad
  • NOTE: HOSPITAL COPIES AND PHOTOCOPIES ARE NOT ACCEPTED. A certified birth certificate has a registrar’s raised, embossed, or multicolored seal and the date the certificate was filed with the registrar’s office; which must be within 1 year of your birth. Please also note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes.

Parent/Guardian Identification

Valid state issued driver’s license, identification card, or passport. (If your driver’s license or ID is newer than 6 months bring supplemental documents. For recommended list click here).

* NOTE- If one parent is unable to attend for children 15 and younger, please see directions below to complete a DS-3053.

Proof of Departure

VIEW DETAILS

Travel must be within 14 calendar days or 30 calendar days if a visa is required. 

ACCEPTABLE PROOF OF TRAVEL:
Copy of computer generated airline ticket, travel itinerary, travel agent generated itinerary, or hotel confirmation              
                                               OR
If traveling by personal vehicle to Mexico or Canada and you are staying with family/friends, please provide an original letter from the applicant detailing your international travel plans.

If traveling for a medical/family emergency please provide a letter explaining in detail your reason and destination address.

1) YOU WILL NEED THE CHILD’S SOCIAL SECURITY NUMBER IN ORDER TO COMPLETE THIS FORM
2) THIS FORM MUST BE COMPLETED & CREATED ONLINE. COMPLETE FORM, CREATE FORM, PRINT FORM.

Tips for completing form:

  • Choose COMPLETE FORM ONLINE in the lower left-hand corner
  • When you come to fees, choose “Expedited at Agency” and “Standard Delivery”.
  • NOTE: FEES WILL NOT BE COLLECTED ONLINE.
  • On final page, click CREATE FORM to print your application. DO NOT SIGN YOUR APPLICATION.

NOTE: If one parent is unable to attend: 

  1. Form DS-3053 (Parental Consent) must be filled out and NOTARIZED by non-applying parent. Be sure there are no scratch-outs, and that the date of notarization matches the date of the parent’s signature.
  2. Include a clear copy of the front and back of the non-applying parent’s identification shown to the notary at the time of notarization (i.e. valid driver’s license, passport)
  3. This form must be recently completed and original. Photocopies are NOT accepted.

Application Fee

16 years or older:
$170 check or money order payable to: “US Department of State”.

Younger than 16:
$140 check or money order payable to: “US Department of State”.

In addition to fees above:
A $25 passport execution fee payable by cash or credit card to Government Passport Acceptance Agent.

Photos, money order fee, application review, hand courier service, and overnight return delivery to our office.

ADDITIONAL INFORMATION

Children under the age of 16:
Must submit their certified birth certificate even if they have had a previous passport and must be accompanied by both parents at the post office or courthouse to sign the minor’s application. Both parent’s will need valid identification. However, if one parent is not able to attend, that parent can fill out form DS-3053 (Parental Consent Form).

DS-3053 INSTRUCTIONS:

  1. Must be original, recently completed, and NOTARIZED by non-applying parent.
  2. Be sure there are no scratch-outs, and that the date of notarization matches the date of the parent’s signature.
  3. Include a clear copy of the front and back of the non-applying parent’s identification shown to the notary at the time of notarization (i.e. valid driver’s license, passport)

For children 16-17 years of age:
At least one parent is required to accompany the minor to co-sign the application at the post office or the courthouse.

Adults 18 years and older:
Can sign application without parent or guardian present.

Instructions

Please “View Details” for each step, gather all of the items, and bring them to our office before 3:00pm Monday-Friday.

Both parents and the minor must appear before an Acceptance Agent to have signatures witnessed. This is a required government mandated step of your passport application. 

The nearest agent is located within walking distance of our office. We will provide you with complete instructions for this process once we have reviewed  your application.

However, if one parent is not able to attend, that parent can fill out form DS-3053 (Parental Consent Form). In addition to the completed, notarized form, a copy of the front and back of the non-attending parent’s drivers license is required.
